COACH PREFERENCE FORM: Coaches may request specific game days and shirt colors by filling out this form and returning it at the Coaches Meeting. COACH BACKGROUND CHECK FORM: All HotShots head coaches are screened through the Washington State Patrol Background Check. CLICK HERE for a copy of the WSP form & EOCF Declaration form that need to be submitted in order to run the check.
GYM MONITOR AGREEMENT: All coaches are responsible for assigning a team parent (or other responsible adult volunteer) to be a Gym Monitor during practices and games. CLICK HERE for a copy of the Gym Monitor agreement form.
